All applicants MUST have a minimum of 1 year clinical experience.

When coming in for the pre-screening, you must bring the original, and one copy, of the following documentation.  Please note, each document must be copied on a separate page.

CNA, GNA , LPN, RN License (you MUST print your own License from one of these sites)

  • Documents to certify your legal right to work in the United States (Ex: Permanent Resident Card with Social Security Card, US passport with Social Security Card, Unexpired Driver's License with Social Security Card)
  • Drivers License or State Photo ID
  • Current CPR I.D.
  • Current 2 STEPS PPD or Chest X-ray

Description of 2 Steps PPD - The Three Vist Approach
Visit 1, Day 1
The first skin test is applied and the individual returns in 7 days for the test to be read. If the first test is positive, it indicates that the individual is infected with TB. A chest X-ray and evaluation is necessary. If the individual is asymptomatic and the chest X-ray indicates no active disease, the individual can enter patient care areas.

Visit 2, Day 7
A second skin test is applied on individuals whose first test was negative at 7 days.

Visit 3, Day 9 or 10
The second test is read. A positive test 2nd test indicates TB infection in the distant past

  • Current Pre- Employment Medical/Physical Examination
  • Hepatitis-B Vaccine Record or Sign a declination form 
     
  • National Background check (LifeMatters can run this for you for a $15.00 processing fee)
  • Current Auto Insurance Documentation (if you drive)
  • 2 Employment Reference and 2 Personal References (with complete contact information)
  • A score of 80% or higher from our Caregiver Assessment Test
